The whole chain a wall adapter actually contains: a 10:1 transformer drops 120 Vrms to 12, a fuse guards the secondary, four diodes rectify both half-cycles and a 470 µF reservoir smooths the result to about 15 V DC. The transformer is a real mutual-inductance model, so leakage droop is genuine, not assumed.
